Maybe a little bit early, but I did receive the following information
from Ray:

*Seed Permit for US members

*The form is attached as a PDF document

*Filling in the form

*In field 1, name and address, telephone.

In field 2, check "New Permit"

In field 3 (Plants or Plant Products to be Imported):
On the first line write "Small lots of seed program" .
On the second line write "Eligible taxa."

In field 4,  check "Propagation."

In field 5, check "Air Mail or Air Parcel Post."

Sign and date and send to:

USDA-APHIS-PPQ, Permit Unit, 4700 River Road, Unit 1236, Riverdale, MD
20737-1236.

There will be an official set of instructions on page two of the permit.

*Using the permit

*When you get the permit, keep the original in a safe place.

Send a photocopy of the permit along with stickers you will receive to
Eric along with  your seed request and Name/Address.  This should be
explained with the permit.
