Don't know if this is the approved way to do this but here are links to
some photos.

First photo shows consanguineum seedlings.  One seedling has 3 of these
extra leaves (Marked as 'A'), and another has 1 extra (Marked as 'B') .
The shape can be seen nicely at this last one.  Basically, it is a
long tapering blade with no distinct seperate petiole.

Second photo shows a new one I just noticed on a candndisimum seedling.
Like the flavum, it appears to be a bit more like the eophyll in shape.
